If you use the Plan de Salud Menonita mobile application and wish to have your account deleted, you may request deletion by contacting Plan de Salud Menonita directly.
Plan de Salud Menonita will process your request and delete your member registration in the app, your login credentials, and related mobile application access.
How to Request Deletion

- State that you wish to request the deletion of your mobile application account.
- Provide the necessary information to validate your identity and membership.
- Once your request is verified, Plan de Salud Menonita will remove access to your app account.

What data will be deleted?
- Mobile app user registration record
- Username and login credentials associated with the app
- Access to the member account within the mobile application
What data may be retained?
Deleting your mobile app account does not cancel your health plan membership or remove records that Plan de Salud Menonita must retain for legal, regulatory, administrative, security, fraud prevention, or healthcare operations reasons.
If you remain an active health plan member, you may re-register in the future and create a new app account.
